2011 Volunteer Application and Disclosure & Consent Form
The American Cancer Society's Hozhoni Weekend is seeking volunteer counselors to assist with the 2011 Hozhoni Weekend Camp for children with cancer and their families. For those of you who have experienced being a Hozhoni volunteer, we hope to work with you again this year! For those new to the experience, we appreciate your interest and welcome your participation!
The camp will be held September 9th, 10th, and 11th, 2011. As usual, we are fortunate to be able to utilize the Wisconsin Lions Camp facilities in Rosholt, Wisconsin (near Stevens Point). All new counselors will receive a packet of information including directions, training materials, and pertinent documents once applications have been received. A mandatory counselor meeting will be scheduled for Friday, September 9th at 4:30pm.
To volunteer for Hozhoni Weekend you must complete a Volunteer Application, a Background Check Release Form and Confidentiality Release Form.
